Replacement Diploma

You can request a replacement diploma if:

• Your diploma has been damaged, destroyed or is irrecoverably lost

• You have changed your name

To request a replacement diploma, please submit an image of a notarized request via email from your Harvard Business School email/LEFA account to registrar@hbs.edu. Please include:

• The reason for requesting a replacement diploma

• Your diploma name and year of graduation

• Your shipping address and contact phone number

• Your signature

• The original diploma (if the request is made due to a damaged diploma or a name change)

The cost for a replacement diploma is $148. Please note that the replacement diploma will not be an exact replica of the original. It will have a small paragraph at the bottom that formally references its replacement nature.
